After your death, coverage under the following benefits programs may
be affected:

| What Your
Dependents May Need to Do |
Your
dependents can choose to continue to receive dental coverage through
COBRA for up to 36 months. Your surviving spouse can continue medical
coverage at the full cost indefinitely, and your surviving child can
continue to receive medical coverage at the full cost until he or she
turns age 23. |

| When They
Should Do It |
Your
dependents have 60 days to elect to continue coverage from the date they
receive continuation materials in the mail. |
|
| How to Do It |
Wait
to receive the continuation materials in the mail from WageWorks.
Complete the continuation forms and return them to WageWorks. |
|
| Special Notes |
If
your dependents do not choose to continue coverage, their medical and dental coverage
stops unless you were hired before July 1987. If you were hired before
July 1987, their coverage extends by one month.

Short-Term Disability and/or Long-Term Disability

Short-Term Disability coverage and Long-Term Disability coverage stop on

Group Life Insurance, Personal Accident Insurance
and/or Travel Accident Insurance
|

Your
coverage stops on the day you die, but a benefit may be payable to your
designated beneficiary. The Benefits Office will notify the insurance
carrier and the carrier will then contact your designated beneficiary. |

Flexible Spending Accounts
|
|
| What Your Dependents May
Need to Do |
Your
participation in the Flexible Spending Accounts (FSAs) stops on the day
you die. However, your surviving spouse or estate may continue to submit claims for
the expenses that you or a dependent incurred prior to your death. |

| When They Should Do It |
Claims
can be submitted up to June 30 of the following calendar year. |
|
| How to Do It |
- Contact the Benefits Office or WageWorks
at 877-924-3967 to request a claim form, or click
here to download the form.
- Submit a copy of your Explanation of Benefits from the
plan for health care reimbursements or a copy of your dependent
care expenses for dependent care reimbursement with the
Flexible Spending Account claim form to WageWorks
— the FSA administrator.

Qualified Transportation Program

| What Your Dependents May
Need to Do |
Your
participation in the Qualified Transportation Program stops on the day
you die. However, your dependents may continue to submit eligible
expenses for services provided to you prior to your death. |

| When They Should Do It |
As soon as possible. |
|
| How to Do It |
- Contact the Benefits Office or WageWorks at
877-924-3967 to request a reimbursement form, or click
here to download the form.
- Submit a copy of
receipts and appropriate documentation with the reimbursement form
to WageWorks — the QTP administrator.

Retirement Benefits

| What Your Dependents May
Need to Do |
Your dependents should
notify TIAA-CREF (800-842-2776) and Vanguard (800-523-1188)
of your death. TIAA-CREF and Vanguard will, in turn, notify
your beneficiaries that a benefit is payable to them and will
assist them in completing the requisite paperwork. If you participated
in the Retirement Income Plan for Employees ("ERIP"),
your dependents also should notify the Benefits Office of your
death so that your benefit under the defined benefit part of
ERIP can be paid to your beneficiary. |

soon as possible after your death. |
