The steps you must take to apply for disability benefits vary, depending on whether you are:

 To Apply for Short-Term Leave:

To find out what you need to do for unpaid leave under the Family and Medical Leave Act, click here.

  • Contact your departmental administrator to arrange your leave and find out how your benefits will be affected during your leave.

 To Apply for Long-Term Disability (LTD):

  • Contact the Benefits Office as soon as possible during your short-term leave to complete the required paperwork for Long-Term Disability.
  • Complete the UnumProvident Long-Term Disability application, have your doctor complete the physician's section, and submit the application to the Benefits Office. UnumProvident determines whether you are eligible for LTD benefits.
  • If UnumProvident approves your application, your LTD benefits will begin after three months of continuous disability. To see how your benefits are affected when you are on LTD, click here.

 To Return From Disability Leave:

  • If you recover from your disability, notify your departmental administrator and the Benefits Office of the date you'll return to work.
  • Submit a statement from your doctor, saying you have fully recovered and are able to return to work.
