You Legally Separate, Get Divorced or End
Your Domestic Partnership
Steps you need to take vary, depending on whether you:
If You Legally Separate or Get Divorced
- Contact your departmental administrator. Ask him or her to
have the following changes made:
- Marital status in the payroll system;
- Name, if applicable.
- Contact your departmental administrator for a new W-4 form
to change your tax-withholding status as soon as your status
changes.
- Complete and return the form to your departmental administrator
within 10 days of your status change.
- Notify the Benefits Office within 31 days of your legal
separation or divorce. If you don't terminate your spouse's
coverage, you may be held liable for any medical and/or dental
claims he or she may have incurred.

If You End Your Domestic Partnership
- You must complete the Domestic Partnership Termination form
and return it to the Benefits Office within 31 days. If you terminate
your domestic partnership, you will not be able to register
another domestic partner for one year from the date of termination.
