You Take a Leave of Absence
The steps you must take vary, depending on whether you're:

If You Take a Leave of Absence
- Contact your departmental administrator to request a leave of
absence form at least three months in advance, if possible.
- Complete
and submit the form to your departmental administrator. Be sure to
specify the reason for the requested leave and the duration.
- Indicate
on the form which benefits you choose to continue.
- Staff
Employees Only: Wait for the Office of Employee/Labor
Relations to approve your leave.
- You must exhaust your
personal and vacation time before your leave of absence officially

If You Return
From a Leave of Absence
- Notify your departmental administrator of your intent to return to
work one month before your scheduled return date.
- If your
position was not guaranteed when you left, contact Employment Services
at least one month before you intend to return to work to see if a
position is available.

If You Take a Leave Under the Family and Medical Leave Act
A Family Medical leave runs concurrently with Short-Term Disability, so
all Short-Term Disability leave will be counted toward your annual allotment
of Family Medical Leave.
- If you're a staff employee who is on
Short-Term Disability, contact Renee
Lucido in the Office of Wage and Salary Administration.
- Contact the following office to request
the Family Medical Leave of Absence Request form:
- Staff Employees: Office of
Employee/Labor Relations
- Academic
Employees: Provost’s Office
- Complete and return the Family Medical
Leave of Absence Request form to the office listed above along with a
completed Certification of Health Care Provider form from your health
provider, if you're taking a leave for medical reasons.
- Wait
for the Office of Employee/Labor Relations or the Provost’s Office to
notify you that your form has been approved.

If You Return
From a Family Medical Leave
- If you're returning to work early, contact your departmental
administrator to let him or her know.
- Submit a note from your
physician along with a written release from your physician, if you've
been on leave for medical reasons.
