 Medical and/or Dental

What You
Should Do
  • You need to change your address with your medical plan.
  • If you're an HMO participant, you should also contact your plan's Member Services department to see if your new residence is within your Primary Care Physician's (PCP) service area.
  • If you move out of his or her service area, you may need to change your PCP.
  • If you move to an area where your HMO does not have providers, you may need to change medical plans. You cannot drop dental coverage during the calendar year except you must drop a dependent spouse in the case of a divorce or a child who has attained age 23.

When You Should Do It If you move out of your plan's service area, select a new medical plan within 31 days of your move.

If you need to change your PCP, contact the plan directly to change providers by the 15th of the month, so the change is effective on the 1st of the following month.


How to Do It Contact the Benefits Office to request the appropriate change of address form(s) as soon as possible.

Complete and return the form(s) to the Benefits Office.

Contact the plan directly to change your PCP.


Special Notes Maroon Plan, MetLife Dental Copay Plan and MetLife Dental PPO Plan participants can see any provider they wish, so participants do not need to inform the plan if they change providers.

 Flexible Spending Accounts

You need to change your address with WageWorks — the Flexible Spending Account administrator.

When You Should Do It As soon possible after you move.


How to Do It Contact the Benefits Office for the appropriate form or click here to download the form. There is no change of address form. Instead you indicate your new address under the "New Address" section on the claim form. Complete and return the form to WageWorks.

 Qualified Transportation Program

 Retirement Benefits

What You
Should Do
To ensure timely receipt of your quarterly statements, benefit payments and other important communications, it is important that you change your address with TIAA-CREF and/or Vanguard.

When You Should Do It As soon as possible after your move.

How to Do It
  • Contact TIAA-CREF (800-842-2776) and Vanguard (800-523-1188) to report your change of address and obtain any requisite paperwork.
  • Complete any required forms and return them to the Benefits Office.
