Retiree Medical Program Rates
(effective January 1, 2008)
The Retiree Medical Program is available to employees age 55 or older who retire
from the University and their eligible dependents. Monthly rates
for the Retiree Medical Program vary depending on the number of
people that are being covered, their ages, and the plan that is
chosen. This screen describes your:

If you are now younger than 65, you must do two things several months before you turn 65:
- Call 1-800-772-1213 to enroll in Medicare Parts A and B
- Send a photocopy of your Medicare card showing Part A and Part B coverage to the Benefits Office, 956 East 58th Street, Chicago, Illinois 60637, Attention: Retiree Medical Plan. Your monthly premiums will then be adjusted to reflect the lower, 65-and-older rate.
Note: Medicare-eligible individuals must be enrolled in both Medicare Part A and Medicare Part B to be enrolled in the University of Chicago Retiree Medical Plan. |

How
to Pay
You may to choose to pay your contribution
rate in one of several methods:
- If you would like to have your premium
deducted from your monthly ERIP defined benefit pension check, contact
the Benefits Office to complete a written authorization.
- If you would like to be billed directly on a monthly or quarterly
basis, contact the Benefits Office.
