Participating in a Medical Plan
You are eligible for coverage under a medical plan if you are a:
 | Full-Time Benefits-Eligible Employee of the University of Chicago (the “University”).
|
 | Part-Time Benefits-Eligible Employee of the University. |
You can also cover your eligible Dependents, including:
 | Employee’s legal spouse or university-registered same sex domestic partner ("DP")
|
 | Unmarried child of employee or DP** up to age 23
|
 | Unmarried child beyond age 23 if the child is:
|
 | Incapable of self-sustaining employment due to a mental or physical disability that occurred before attaining age 23;
|
 | Dependent on employee/DP for support and maintenance; and
|
 | Covered continuously by the plan prior to and beyond age 23
|
 | A child is a:
|
 | Natural child
|
 | Stepchild
|
 | Legally adopted child
|
 | Ward
|
 | Any individual named in a court order requiring coverage for a child (QMCSO)
|
| Who meets the following criteria:
|
 | Employee/DP has legal guardianship of child;
|
 | Child lives with employee/DP in parent/child relationship; and
|
 | Child provides no more than 50% of his/her own support
|
**If the domestic partnership terminates, a child of the DP would no longer be eligible for coverage under the plan.
You cannot provide coverage for your parents; grandchildren who do not meet the requirements listed above, or foster children.
Verification of relationship is required for any dependent being added for the first time to your coverage. Documentation may include a marriage certificate or domestic partnership statement to add a new spouse or domestic partner. Documents for a new dependent child may include a birth certificate or adoption papers, or other appropriate legal documents may be provided. Return a copy of the required documents to the Benefits Office within 31 days of your date of hire as a Benefits-Eligible Employee.
If you and your spouse or Domestic Partner both work for the University, only one of you may cover your children as dependents.
