You can use the SFAP if you are a University:
- Employee or retiree; or
- Family member of an employee or retiree.

- Family, marriage or work;
- Financial or legal;
- Emotions and stress; and
- Alcohol and drug abuse.

How SFAP Works
A Perspectives counselor will meet with you or your
relative to assess your situation and help to develop a plan. This plan might include:
- Short-term counseling with a Perspectives counselor; or
- Referral to a resource such as a self-help group, a social
services agency, or a specialized counselor.
In addition, you have 24-hour access to an online tool that provides
a library of articles, self-guided assessments, links, calculators,

Confidentiality
Confidentiality is an important aspect of a Staff and Faculty Assistance
Program. The University is dedicated to providing you with a secure
and private atmosphere in which to seek help. What you discuss with
Perspectives staff will not be shared by them, except as may be
required by Federal and State laws. No record of your participation
will appear in your personnel file. In an instance where the referral
is work-related, with your written permission, your supervisor may
be told whether you have followed through with your appointment.

Perspectives is located on campus and in many locations throughout the Chicago area and northern
Indiana. Call Perspectives at 630-932-8008 or 800-456-6327 to find out which
location is most convenient and to schedule an appointment.
Emergency consultation is available 24 hours per day, seven

Cost of the Program
Use of the SFAP is free. If you need referral services or
a treatment program, Perspectives will work with you to obtain the best care as
inexpensively as possible.
