Leave of Absence
Purpose
Used to cease employee's
compensation for up to one year but to maintain service date.
- Provide continuation of Benefits
- No break in service if reinstated back to position
Form
(#UPP 167) is used for Regular Staff
Employees.
All requests approved/denied
by Employee Labor Relations.

of Absence:
- To attend school
- Personal
- Military Service
- Medical Absence
- Other, specify
